SAFETY FIRST All clients must fill out a Client Intake form and Policy/Security Agreement form prior to receiving therapy. This is because some of our services are not recommended for people with certain conditions. You will fill out the forms at the time of booking, unless booked by someone else or not online. In this case, you will need to fill out the form at the time of your arrival. Please make sure you click on the appropriate form (review your booking info first to see which form to use, and make sure you select the right massage therapist). Please note: Massage tables & Float Tub include a 350 lbs weight limit. We fully uphold Massage Therapy and all of our spa services with the utmost respect, and we take our licenses and professions very seriously. We have a zero tolerance policy for abuse or harassment of any kind, especially sexual abuse or exploitation. We will hold any guilty party responsible to the fullest extent of the law. Please be aware that the Sheriff's Office is across the street, and they will be notified if needed.
